Quest Name: Bertoxxulous and the Throne of Plague Level: Class: All Quest NPC: Location: Plane of Disease
Bertoxxulous, also known as The Plaguebringer, has no allies. His followers often follow the dark art of necromancy, which he wholeheartedly supports. He appears as a humanoid hunchback draped in tattered, decaying robes. Under his hood can be seen a handsome face attached to the body of a rotting corpse.
Bert is located through the same portal where you'll find the Twins (Poxathixius and Uzrathixius) in the Plane of Disease (PoD). He is a quest spawn, generally he can be spawned every 3 days (24-96 hours).
If you can con "throne of Bert", he's spawnable. You'll need to take your stinger that you looted from Queen Qy'tha to an npc near the Queen's portal (Witchdoctor Muglchlur). He'll exchange your stinger for a scepter (but only if you have positive faction with the pulsings-TY Athonasius). Take the scepter to the connable "Throne of Bertoxxulous" and you can spawn him.
He has a 6k crit (which sometimes goes off twice!) and quads with 1500-2200 a melee, has numerous AoE's and DoT's, and debuffs both DR & stamina by 125. If he's holding the Katana of Severing, it has a 2k proc. You get hit with a disease spell just swimming across the lake towards him. You'll need an army of patchers because he is one tough mob to kill.
Check under Loot table for what he drops, he also drops 2 dust for the PoD ability quest and has a chance of dropping a PoD spell.
Quest Name: Noroxus Portal Quest Level: 50+ Class: All Quest NPC: Honcho Oogurlgze Location: PoD
You need to kill Noroxus to enter the twins area, in Plane of Disease (by looting his blood). He's not a mob to be taken lightly. One of the hardest hitting mobs in the game, at the moment. Strategy on killing Noroxus below.
Speak with Honcho Ulgurglch in POD to begin quest (he is marked on this map-he's almost all the way around the circle clockwise). While running thru PoD stay high up on the canyon walls as many mobs see through invis. Note: Noroxus must be up in order to log the quest! Otherwise Honcho will tell you: "Noroxus has been defeated. Close eye on Ulgurglch masters are keeping. Ulgur no can speak for now."
In general the quest is to bring 3 hearts back to Honcho.
So, Questor goes to each of the 3 pools (no order necessary) and does the following:
Return to Honcho with all 3 slime hearts.
Honcho does some magic and gives you an item back. Take the combined item to the big cauldron in the middle of all 3 pools to open fuming portal. There are toxicologists surrounding the cauldron that must be killed. Once the portal is open, all people with a skin can go through. Portal stays open 10-15 minutes, don't open until everyone is ready to go through.
This leads to Noroxus. Kill nox and get blood of Noroxus. Don't forget to coh in a low level toon to leave behind. This way you can load your spy and check whether Nox is up. Everytime your guild wishes to do Nox, they must complete steps one through seven.
Strategy
All necs stack down by the rocks near the water (to the S/E of nox). Necs stack on one side of rocks, Tank stands beside rocks. Necs get los of tank, but stay out of aoe. The strategy can best be summed up in these immortal words:
"If you're not in los, get fukkin in it and if you're in the AoE, get fukkin outta it." Fredjones 2006.
Check pacts on tank, make sure they'll go off. Make sure all patchers get batteries to support them or they will go oom. Have another tank or healer pull noroxus to the tank (MT can't move or will lose los for the patchers). Pets passive, and no one attacks until MT has aggro from puller.
Its safe for all melees to strike from behind nox, his AoE goes only forward.
Reward:
Noroxus Blood: As long as the closed portal itself is spawned, you can use Noroxus Blood to open the Bloody Portal and pass through to the twins in PoD. Unlimited use (don't delete).

PoD Drop Spells: You used to have to complete plague quests for these spells, and depending on your alignment (good/evil) would get a different spell. The quests have now been removed and placed in the random drop table off Bertoxxulous, noroxus, or the twins. These spells are now the only PoD spells available:

Quest Name: Plane of Disease Portal Quest Level: 50+ Class: All Quest NPC: Necrologist Zizclank Location: Dead Hills (NW)
The purpose of this quest is to get 'flagged' for the Plane of Disease (PoD) so that you can port there anytime.
First, get a Zombie Flesh Suit, you can have one crafted for you, but it's easier to buy one off auction. Try and get one for less than 400k, watch the auctions for a few days and you should be able to do this.
Rather than log the quest, it's more efficient to collect the three Death Knight Hearts first (NTL), then take them with you when you go to log the quest. None of the three Deathknights are triggered spawns.
For the first Death Knight, run SW from the Oasis coach to Takish Hiz (W). Go past it on the west side to the dead trees on the other side. This mob spawns randomly among the dead trees. Deathknight Endraval see's through invisibility. Kill him and loot his heart.
For the second Death Knight, run straight South from the Castle in CLW into Salisarean (N). Keep going south and you'll find a rift surrounded by dead trees. One end of the rift is in Salisarean NE and the other end of the rift is in Salisarean E. In the rift at each end, Deathknight Bileheart spawns (sometimes side by side). One is light blue to a 60, the other is yellow, both are easy to kill. You only need to kill one and loot his heart, it doesn't matter which one you kill.
For the third Death Knight, run straight South from Black Water to the border of Fort Alliance Center and Fort Alliance South. Then go west, and you'll find a small circle with rune stones. Kill DeathKnight Halik and loot his heart.
Now take all three hearts and run to Dead Hills (NW). Log the quest by speaking to Necrologist Zizclank and his assistant Necroloigst Xeptip. Then you want to enter the mausoleum in the middle. Kill the mobs out front, and enter the mausoleum. Kill mobs on your way through to the portal. Then port through to the sanitorium.
Once in the sanitorium, you'll want to speak to Fatima. As you talk to her the Orderlies that previously conned white-faced will now attack you, at least 2 or 3 will. Kill them and go where Fatima tells you to, to the end of the hall with the 3 bathtubs. As you approach, you'll get a popup. Kill the mobs and loot the healer.
Now, head past Fatima and find Rebo. As you talk to Rebo you will be attacked by Orderlies again, kill them and go back to Fatima. Fatima now sends you to kill the pharmacist and loot the wasp venom. This part is tricky, the questor MUST be the same person who talks to the pharamicist and then loots. Make sure you don't spawn the quest, but another in your grp loots or you'll have to delete quest and start over. Be in the group that kills the pharmacist.
Go back to Fatima, and she'll send you to Medelonia, where you might be attacked by orderlies again. From here you must go to the Last House physician and kill him and loot his robe. Off this room is a small hallway, talk to the orderly at the end and he ports you to the other side of the wall.
Once on the other side, be careful, go up the ramp and down the hall to the left, then left again and down a hallway that will take you to the portal. Take the portal, and CG! You're now flagged for the Plane of Disease. Note on using the portal: Enter the last room, ONE at a time. Do not con/talk to the portal. As the first person approaches the portal you'll get a popup, chose the option to pass through. Only then should the next person enter the room and approach the portal. Using this technique to enter the room will allow everyone to get through without delays, whereas having numerous toons entering the room all at the same time seems to make the portal glitch and it takes a long time for each person to get the popup.
ALWAYS take your fleshsuit with you when you want to port to PoD, but you will no longer need the Last House physician robe.
Note: It can take a while to port a large group through, the port is on a timer, just be patient.

Quest Name: Yarfsnoogle Level: All Class: All NPC: Yarfsnoogle Location: Plane of Disease
Quest is logged by an NPC named Yarfsnoogle in the Noroxus pools area in the Plane of Disease (Last Home E) . He's across the valley, straight south of Honcho. Lots of roamers around NPC, approach him carefully. You are told to gather and bring back EIGHT different anatomy structures. Fetid rotting: femur, tibia, skull, organ, soul dust, eyeball, skin, and gland. Note: You do not need to LOG the quest in order to begin farming for the items.
Nine mobs each guarantee one unique quest item drop; a primary quest item. These nine mobs have a %chance to drop a second random quest item on top of their already guaranteed primary quest item, which may or may not be the same as the primary. It is unknown yet if this secondary drop is random or is set like the primary. Right now, it looks totally random.
So in essence, you can obtain these quest items by doing the raiding you and your guild have normally been doing (Noroxus, Bertoxxulous, etc.). Or you can accomplish this task by grabbing a good group and seeking out and farming the rare spawn mobs for their primary drops, as well as farming them enough to start obtaining the secondary random parts you need. In other words, you can spawn and farm one of those mobs and eventually obtain all 8 quest items within time thanks to the random second item.
The Dust is the hardest item to obtain and as of the April/07 update you can now take all 7 of your items back to Yarfsnoogle's pet, and he will give you a bag (to hold your seven items), then you can safely store the bag in your bank while you work on farming your dust. Once you have the dust, take the bag and dust back to the pet who will restore all 8 items to your inventory, then you can talk to Yarfsnoogle to complete the quest. This way you don't use up 7 inventory spaces waiting for the 8th item to drop.

First, inside the second valley just through the tunnel with the banker; and second; continue to the end of this valley, enter the tunnel and head to the third valley, he spawns in this one also. If you haven't seen him in the first valley for a while, try killing plague swines in the second valley; or, he could be up in the second valley which prevents him from spawning in the first valley. HOW TO SPAWN:
NOTE Near the portal where you come to PoD, across the first pond, is a series of tents. In one, you'll find Tuskmaster Helgley who will give you a hunt and gather for plagueswine tusks (xp is about 250k +956 tunar per four tusks; sadly no faction with pulsings). Difficulty: easily killed with a decent group, no longer sows and runs at end.
Location1: Clearing jelly's/piranah's will spawn db scuzz toads; killing blues will spawn whites, whites will spawn yellows. Killing a yellow scuzz toad gives you about a 40% chance of spawning Glub (who'll spawn right on top of you). The entire cycle (from db->white->yellow->Glub) varies, and can take anywhere from 15 minutes to 45 minutes. Glub runs at 1/2 red or so, and stays in the water; in order to avoid getting adds, make sure you keep some big spells to wipe him out quickly. (SKs use darkness, sometimes it will slow him down [DR resisted though])
Location2: The second lake works a bit differently, you try and spawn RED scuzz toads, and when you do, you are guarenteed that glub will spawn upon the death of the red. But killing a yellow toad, does not guarantee a red one will spawn.
Difficulty: easily killed with a decent group. NOTE You'll stack a lot of scuzz toad eyes doing this, you can turn them in by speaking to pluxklez (he is in the same room where lich overlord spawns). For every four you get 251000xp and scuzz toad stew, lvl 45 (eat it, and you get a ticking heal for 156)

For the easiest farming, we suggest the following: Skulls, skin - FLESHRIPPER Organs, glands, eyeballs, tibia - GLUBOXIOUS THE FETID Femur - RALLIUS RATTICAN These mobs are the easiest to spawn/farm for the drops they give.
Reward
Upon returning to Yarfsnoogle with the 8 items, you receive your class specific PoD ability that is NO TRADE LORE. Also, an NPC named Grum spawns and attacks you. Grum will drop an item specific to the class of the questor. Many of these items have DR based DS.

Tanks max DR/PR
The Plane of Disease Weapon quest is not that complicated. First, you log the quest with Witchdoctor Yurplag. To find him, once you port to PoD from the Last Home, go north and take the first tunnel on your left. Follow that tunnel through to the end and go left. Follow the mountain around and stay high on the mnt to avoid aggro'ing the wildlife. Not too far ahead, on the other side is another tunnel you must get to. Head into that tunnel and follow it through, head right out the tunnel and stay high on the mnt. Follow the mnt for a long time, eventually crossing to the other side. Stay high up on that side until you reach the tunnel that leads to the area of the PoD Weapon Quest. See Map for details.
After exiting this tunnel, right below is the NPC Witchdoctor Yurplag. Log the quest. He asks you to bring him:
Note on the map, there are specific quadrants to farm for each of the four mobs. First, you start off killing the Larvae (con db and lb at 60). Contine killing larvae until the mid-level mob spawns (bigger, and con white/yellow/red to 60). Eventually the Matriarch/Monarch will spawn. All mobs are one groupable at level 60. The entire spawn cycle from larvae to boss is variable. CS found it can take anywhere from 30 minutes to two hours to work your way through cycle. Each mob drops only 2 items. So the quest involves a commitment to a lot of farming until everyone gets their required four drops.
It seems things would go smoothly if we started with locust, proceeded clockwise to the millipedes, then moths, then mantis. However, if one of our party just needed moths, and we started killing larvae there, sometimes the quest would proceed through the different mobs, sometimes the larvae would stop spawning completely and wouldn't spawn back for over half an hour. Relogging the quest didn't seem to help much.
Sometimes getting the big bosses to spawn can seem tricky. CS noted, that if you regularily only pull the same one - two mid level mobs, at some point one will NOT spawn back. This is your sign that the boss has spawned somewhere nearby, take a careful con and you'll find him. He's on a timer, so don't wait too long to engage him or he despawns. Sometimes, where you are farming can be quite aways away from where the boss will spawn. This is why containing your farming of the mid-level mobs to the same one to two is the best way to alert you when it's spawned. The boss always spawns in the same location (for that type).
Also, if you get to the mid-level mob point, and take a break (say tank deathgates to repair), they will despawn and put you back at the larvae stage. So when your group needs to break, do it after killing a boss. After you've killed a boss, that area seems to tire out and larvae won't spawn back for a while. Best to move onto a different mob, and rotate among the four areas. We tried logging/relogging quest but that never made a difference for us.
Returning the four pieces back to the Witchdoctor grants you some small xp and the following rewards. Most who did this quest felt the caster item was great, and worth all the trouble, but for other classes our 60 epic wep's were better.

From the port spot you should be able to con Witchdoctor Muglchlur, talk to him to log the quest for the queen's stinger. Just past him is a lake.
In the center of the lake is a portal called "whirling portal", clear the mobs in the area and talk to it. The portal will talk to you and advance your quest.
After you talk to the portal go to the north side of the lake and talk to Bonefisher Chklurgl inside the tent to advance your quest.
NEXT PART REQUIRES A SACRIFICE BY A PLAYER WITH QUEST LOGGED. Someone must swim over the portal on the surface of the water who has the quest in right spot. You will get a pop up that asks if you want to "inspect the mud", say yes. At this point 4 Filth Squids will spawn and all mob you, you must zerg your way back to shore (having max debt ahead of time is less frustrating). Everytime you rez face south and press the controller forward durring the loading screen. Since the squids root it takes 3-6 tries to escape their aggro and make it to safety. They can be nicely single pulled with low level nec life tap.
While you are waiting for the named kracken to spawn, Sthlumchoxius, kill all the mobs around the portal.
Sthlumchoxius spawns about 10 minutes after you trigger the Filth Squids. He can be single pulled easily if you decide not to kill the mobs around the portal. He conns red to a 60 and is fairly doable by a group of 4 with tank and healer if high enough level.
THIS IS VERY VERY IMPORTANT! ONLY THE "KILL" GROUP GETS FLAGGED FOR KILLING IT. The best way to work this is to have everyone who needs flagged form a group (if more than 4 need it you have to do this process multiple times), anyone else there who does not need flagged should stay alone, this will ensure that the kill group gets credit. Note: Everyone in the KS group needs to be on the part of the quest where the Bonefisher tells you to kill whatever is in the lake killing the puslings. If you're not on this part of the quest it won't work. Also, although this is a KS group flag, the Aoe for it is very small. When Sthlum runs, everyone in the group needs to stick to him like glue or you'll miss your flag. If you got the flag, your quest log should update and tell you to see the bonefisher again. Whe get flagged: At the end of the quest, you get the queen chitin, this allows you to always pass through the portal to the queen without having to do the quest again (you must have the chitin in your inventory (not in bank) each time you wish to pass through and kill the queen).
Once you are flagged as killing it, return to the Bonefisher and he will reward you with 500xp and a chitin. The item he gives you allows you to speak to the portal and be ported to the wasp hive. Keep the Chitin, you can use it again the next time you wish to do the Queen. WARNING: it is likely you will port into a red conn mob (Tormented wasps), or worse yet the queen! Make sure everyone is going before you take the trip. Once you are there there are two ways back, one is to RH and the other is killing the queen to open a portal. Run yourself south to a safe spot.
Now you want to farm the Tormented Cadis until the Queen spawns (loot your kills for faster respawns). Takes about a half hour of farming, or sooner if you're lucky, for the Queen to spawn and object to you killing her children.
The Queen can be easily 2 grouped. Last time we did had DK, resist ff cleric, druid, 1xnec, 2xmage, bard, and alch. She has an AoE, so ensure your party is shielded so they don't get wiped. Once you've killed her, you can wait on the inside for the portal to despawn and begin farming again, or leave through the portal and porta back inside to reset the zone. Reward: She drops ONE stinger for the witchdoctor quest. Taking the stinger back to Witchdoctor Muglchlur will allow you to get a totem that can spawn Bertoxxulous when his throne is 'up'. She'll drop an eye for the PoD ability quest. Loot Table
The Twins, are twin dragons, Uzrathixius and Poxathixius. They are Bertoxxulous' pets in Plane of Disease. See map here for location. They are random spawns, not quest spawned.
They are farmed primarily for the PoD drop spells they occassionally drop.
You can only get through to the Twins by having Noroxus' blood. You get this by killing Noroxus (see quest here). So do Noroxus first, then twins.
If the twins are up, get to PoD and head all the way around the circle past Honcho Ulgurglch to the Closed Portal (sometimes referred to as the bloody portal). Do not open the portal until all your raiders are assembled and ready to pass through (portal will only stay open for about 15 min). The respawn on the portal is random, anywhere from 45 min-3 hours.
Stay away from the edges of the area you port through to, there are death touch wisps (unconnable, but visible) along the sides and across the moat where Bertoxxulous spawns.
Tactics: Make sure your shaman uses Wind/Primal Malady/Umbral Curse to debuff them to ensure they melee for less (cast every time it's up). Note: Shammy debuffs are said to be DR-based, and consequently they may be less effective in PoD, which is high in DR. However, the AC debuff of the 60 shammy epic spell is reportedly non-resistable with DR. The twins spawn to the left and the right, above the rocks, near the moat. Have necs stack behind big round boulder (on moat side), tank stands to left in between the tall rock and round boulder. Get LOS on tank, stay out of AoE.
Twins spawn every 2 days generally, and nearly always respawn after a server crash/reset.
